You must reply to the email to confirm the proposed information. A $200 non-refundable scheduling retainer will be required after you confirm the appointment date, time, and location. The testing date is not secured until the retainer is collected. The remaining $300 will be charged on the morning of your evaluation. Please see the Rescheduling/Cancellation Policies.

Please confirm that you have read and understand the following points:

1. There is a $200 scheduling retainer. The testing date is not secured until the retainer is collected:
2. If my child has been administered the test within the past calendar year, they are ineligible for retesting until a full calendar year has elapsed from their previous evaluation.
3. Receiving coaching or tutoring specifically for this test is strongly discouraged and any indication that this has occurred will be included in the report.
